As a service to you, our office staff will contact your insurance company to obtain pre-certification and authorization for the physical therapy services you receive in our clinic. We will also bill your insurance company for services provided.

The authorization we receive from your insurance company is not a guarantee of payment for services. Your responsibility is subject to individual coverage, co-payment and deductible. We recommend that you contact your individual insurance company for specific coverage and restrictions.

You are responsible for any insurance co-payment at the time of service rendered.

You have the option to pay by cash, check, charge.
